From local innovation to system change – public-sector pathways for scaling community-driven initiatives on noncommunicable diseases

People and communities directly affected by noncommunicable diseases (NCDs) and mental health conditions are strategic partners in shaping effective and equitable responses. Their firsthand knowledge of living conditions, cultural contexts and economic circumstances helps identify needs and implementation priorities. Through their peers and local networks, communities can also contribute to culturally sensitive and inclusive health policies and programmes.
When aligned with national health priorities, community-driven initiatives can be scaled beyond their original settings, but doing so requires support from the public sector and other partners. Drawing on lessons from the WHO NCD Lab’s 2021–2024 project cycles, this technical report assembles initial evidence and practical experience on identifying, engaging and scaling such initiatives. It examines common barriers and enablers and identifies three priorities for wider implementation: strengthening government capacity and leadership, supporting community innovators, and investing in multistakeholder coordination and partnerships. The report emphasizes that scaling efforts should be developed with communities and remain guided by their priorities.
